CUSTOM COOKIE ORDER FORM

Please be sure to review the pricing list to determine the budget you would like me to stay within for your order.  There are multiple tiers for pricing (Basic, starting at $55/dozen, Detailed, starting at $70/dozen, and Elaborate, starting at $85/dozen). Please note, there is a 2 dozen minimum on all custom orders. Once your request is received, please expect to hear from me via email to discuss design and cost. Orders are not processed and added to my calendar without payment of invoice.

Important note! Due to the time and love put into each custom decorated cookie, orders submitted less than 2 weeks prior to pickup date or orders needed for a date I am already booked, will incur a 20% rush fee.
